Performance Evaluation for Classified Employees
and Teacher Assistants

I. PERFORMANCE EVALUATION REQUIREMENT
In accordance with District policy and collective bargaining agreements (CBA), the job performance of all PERMANENT classified employees as well as Teacher Assistants must be evaluated at least once each year on the appropriate forms. For employees in Bargaining Units B, C, F, and S, evaluators must make a reasonable effort to issue the annual evaluation at least twenty (20) working days prior to the end of the employee's assignment.

Annual performance evaluations are NOT prepared for classified employees serving an initial probationary period or for employees with temporary or restricted status.

II. PREPARING THE PERFORMANCE EVALUATION
Prior to preparing annual evaluations, evaluators are reminded to review the performance evaluation provisions of the applicable CBA (see VII below). Failure to follow these provisions may result in the filling of a grievance or appeal of the evaluation. The attached form (Attachment A) or a similar form must be completed when rating in factor "below standards."

III. DISTRIBUTION OF THE PERFORMANCE EVALUATION
The employee must be given a photocopy of the completed evaluation. The evaluator retains the original.

IV. NOTICE OF OUTSTANDING PERFORMANCE
If the employee has performed above the recognized standard for a position, a Notice of Outstanding Performance should be issued. The form should then be forwarded to the Classified Personnel Assignments Branch at 333 S. Beaudry Ave., 12th Floor, Los Angeles, CA 90017 for inclusion in the employee's personnel service folder.

V. ATTENDANCE GRID
For evaluation forms with attendance guides, carefully complete the grid by using the appropriate payroll calendar in combination with your time reporting records. It may be helpful to line out off-track and/or off-basis periods.

Indicate in the appropriate box the number of hours the employee was absent for any reason except vacation, holidays, personal necessity taken for religious holidays and school holidays (recess periods).

VI. RECLASSIFICATION PROCEDURE
Personnel Commission Rule 545 REQUIRES that, in order to be reclassified with a position, an incumbent must be rated overall as "Exceeds Standards" on the two most recent annual evaluations. It is the responsibility of both the supervisor and the employee to ensure that evaluations are given at appropriate times.

VII. EVALUATION FORMS TO BE USED
All regular classified employees and teacher assistants must be evaluated at least once a year. Links to the forms to evaluate Permanent Classified Employees (PC Form 80.22a), Permanent Unit D Employees (PC Form 80.22), Permanent Classified Administrators (PC Form 80.24) and Teacher Assistants (PC Form TA 5219) are provided below. With the exception of the Teacher Assistant evaluation and the "below standards" form, these forms are formatted for legal size paper. Please refer to the charts below for additional information and exceptions.
