Photocopy this form and submit it to the Principal for approval at least two weeks prior to the planned field trip. Teachers are also required to have a Medical Authorization Form for each student. (Photocopy the form below). Concurrent students must also have a Student Consent for Curricular Trip Form signed by a parent. (Forms are available in the Counseling Office.) Teachers should keep the form with him/her during the field trip.
