THREE EASY STEPS TO A SUCCESSFUL FIELD TRIP!

       Step 1: Complete the required paperwork

    Step 2: Receive a Trip Confirmation Number

Our Trip Confirmation Unit will send you via school mail a Trip Confirmation Report to ensure receipt of your completed application. You will, at that time, receive a Trip Confirmation Number.
  Step 3. HAVE A GREAT TRIP!

REQUIRED APPLICATION FORMS

A supply of forms may be obtained by submitting a written request to the Transportation Branch

For trips involving bus transportation, the following documents are required by the Transportation Branch:

Form 78.20T, Application for Auxiliary Transportation/Trip(s)
This form is to be used for all other trips. Applications for specially funded program trips to be paid from School Improvement (SI), Economic Impact Aid-District Bilingual (EIA), and related funds need not be requested because they are automatically sent to the schools by the Transportation Branch Accounting Unit, based on yearly budget allocations.

An additional application form 78.20T must be submitted to Special Education, Room H-119, Administrative offices, at least 30 days prior to the date of the trip.

Form 78.20R, Authorization for Payment of Student Body and/or Reimbursable Trips
This form is to be used for those trips paid by the Student Body Funds or by donations from individuals, Parent Teacher Association (PTA), private and public organizations, museums, universities, etc.

Request for a Lift Bus
A lift bus may be requested for one or more physically disabled students, whether enrolled in a Special Education Center, a Special Education class in a regular school, or enrolled part- or full-time in the regular program.


HOW DO I CANCEL MY FIELD TRIP?

1. If you made reservations for you field trip:

a. Please contact your site and cancel your reservation.
b. Please contact Dispatch at 323-342-1460 or 800-LA-BUSES to cancel your bus.

2. If a reservation was required by your destination and placed by School Journeys:

Please contact School Journeys at 323-342-1342 or 1-800-LA-BUSES and we will cancel your reservation at the field trip destination and help cancel your bus through our Dispatch Unit.

3. If a reservation was not required from your site:

Contact the Dispatch Unit at 1-800-LA-BUSES to cancel your bus.
