NAME BY WHICH YOU WERE KNOWN Enter only if you were known by another name during your tenure at Verdugo (i.e. maiden name, married name, etc.).
E-MAIL ADDRESS Please provide only the primary one you would like to have published.
STREET ADDRESS (Optional) If you would like to only list the city and state (or city and country) you reside in, leave this space blank and enter it in the City, State and Zip blank instead.
CITY, STATE and ZIP (Optional) Provide Country and Postal Code for addresses outside of the U.S.
TELEPHONE (Optional) Specify if this is a work number. Be sure to include country codes for oveseas telephone numbers.

FORMER CURRICULUM or ACTIVITY Please describe the core areas taught at Verdugo if retired or no longer actively teaching. List last position at Verdugo if in administration, staff support, or other capacity.

TO ADD A FACULTY / STAFF LISTING
To add a listing to the faculty & staff directory, please check the "Add Listing" box, fill out the form as you would like it to appear, and press the "Send It" buton to submit the information.
TO MODIFY or DELETE A LISTING
If you are submitting changes to an existing listing, please check the "Modify Listing" box and fill out the form as you would like the listing to appear. To delete a listing, check the "Delete Listing" box and provide your name and an e-mail address for verification purposes. Press the "Send It!" button to submit the change request.

If you choose to send e-mail instead of the form above,
you will need to type the information into the body of the
