LOS ANGELES UNIFIED SCHOOL DISTRICT POLICIES AND PROCEDURES REGARDING STUDENT HEALTH :

 

IMMUNIZATIONS - In keeping with the State of California immunization requirements, new students will not be admitted to school unless a written immunization record provided by a physician or the health department is presented at the time of enrollment and immunizations are up to date. There is now a requirement for Hepatitis B and Measles, Mumps & Rubella (MMR), immunization for new enrollees aged 2 months to 4 years and 6 months.

Students who currently need additional required vaccine doses or who lack a written record of having received the required vaccine doses are no longer allowed a grace period. All students new to the Los Angeles Unified School District or transfer students within the District must show that they have received all currently due required immunizations in order to be enrolled.

The immunization status of all students will be received periodically. Those students who do not meet the state guidelines must be excluded from school until the requirements are met. Students who have been exposed to a communicable disease for which they have not been immunized must be excluded from school at the discretion of the health department.

TUBERCULOSIS - In keeping with the tuberculosis testing requirements of the Los Angeles County-Department of Health Services, all new kindergarten students and all new first grade students who have never attended kindergarten must present a written report provided by the private physician or health department giving the results of Mantoux test for tuberculosis. This test must have been done within one year prior to school entry.

All other new students entering grades K - 12 who have never previously attended any school in California, must present documentation of the results of Mantoux test and Hepatitis B, done at some previous time.

MEDICATIONS - The parents/guardians of a student taking any prescribed medication on a regular basis at home or at school, must inform the school nurse or principal of the medication being taken, the current dosage, and the name of the supervising physician. A student who needs to take medication during school hours must have a statement to this effect on file a the school, signed by the prescribing physician and the parent/guardian. The required forms are available from the school nurse or administrator. School physicians and/or school nurses do not prescribe or give advice regarding medication or other care beyond first aid. We CANNOT administer medication (including aspirin, cough drops, etc.) without this physician's form.

PHYSICAL EXAMINATION - A Child Health and Disability Prevention (CHDP) examination is required for all first grade students 18 months prior to or up to 3 months after admission to the first grade. This examination may be done by a private physician, by a health department clinic, or, in some instances, by the School District's CHDP staff. A blood test to determine lead level is part of this examination. If parents/guardians do not wish to have their child examined at school, they must file an annual written statement to that effect with the administrator .

Failure to provide a CHDP examination by the end of the third month of first grade will result in an exclusion from school up to 5 days.

Screening of vision and hearing will be done in first admission year to school, and thereafter in accordance with state mandates. Parents/guardians will be notified of any conditions requiring further attention.

Miscellaneous - Communicable disease inspections will be conducted periodically. A student suspected having a communicable disease will be excluded from school until guidelines for readmission are met.

The parent/guardian of a student with a short-term health problem which makes it physically impossible for him/her to attend school, should arrange with the school for a temporary plan to continue the student's education.

A student returning to school with a suture cast, crutches, wheel chair or leg braces must have a physician/s written permission to attend school and must comply with any safety procedures required by the administration. A student needing to attend school in a wheelchair must have approval of Health Services personnel and the school administrator.

A student returning to school following a serious or prolonged illness, injury, surgery, or other hospitalization, must have written permission by the health care provider to attend school, including any recommendations regarding physical activity.

An excuse (less than 10 weeks) from a physical education class may be granted to a student who is unable to participate in a regular or modified curriculum for a temporary period of time due to illness or injury. A parent's written request for an excuse will be accepted for up to 5 days; thereafter, a written request is needed from the student's health care provider.

An effort will be made to notify parents/guardians about school exposure to chickenpox. The parent/guardian of a student for whom chickenpox presents a particular hazard should contact the school nurse to facilitate notification. Students at risk include those with deficient immune systems and those receiving certain drugs for the treatment of leukemia or organ transplants.

A current Los Angeles Unified School District Emergency Information card must be on file at the school so that parents/guardians can be promptly notified in case of accident or illness involving their child.

SCHOOL TELEPHONE - The telephone number is (818) 353-1730. Please feel free to call if you have any questions or concerns. Because we have limited telephone lines into our office, children needing to make non-emergency calls are asked to use the pay telephone located in the office. We do not interrupt classroom instruction to relay messages, so we cannot take messages for students unless it is an emergency. Please discuss procedures you want your child to follow should you not be able to pick up your child at the regular time, ie: walk home or go to the playground, etc. The district does permit students to bring cell phone to school for telephone use only. Students are not to use cell phones during school hours, from 7:40 AM to 2:29 PM. Cell phones must be at the off position for the entire school day. If at any time the procedure of using the cell phone is followed, the school will confiscate the cell phone and a parent/guardian must personally come to the school to claim it. The school is not responsible for the loss of cell phones in any capacity. We highly discourage students from bringing cell phones.

ROOM PARTIES - Two room parties per year are planned for our students; one is before the winter vacation and one in the spring semester (these usually last no more than one half hour). Please do not plan surprise parties, birthday parties, or send sweets and drinks to school. And please, no gum at school at any time. No food will be allowed at school unless permission is obtained IN WRITING from the teacher and the principal.

CARE OF PERSONAL PROPERTY - The Los Angeles Unified School District does not provide insurance for loss of, or damage to, personal property of students. Children should not bring personal property to school such as large sums of money, purses, or expensive clothing, backpacks, jewelry, watches, electronic games, radios, backpacks, bicycles, skateboards, caps, hats headbands, etc.

Please see that all of your child's belongings, including coats, lunch pails, paper lunch bags, raincoats, sweaters, and other items are labeled with your child's full name and room number. If this is done, the lost and found can do a much better job of returning lost articles.

SAFETY - The safety of your child is of concern to all of us. The route your child takes to and from school should be carefully selected for the least exposure to traffic hazards. We ask parents to review safety measures for children who walk to school. If you drive your child to school, designate a location where you let your child out and pick him up each day. The parking lot is off-limits for this purpose. Please do obey the traffic signs. DO NOT park in the bus zone; buses need to park next to the curb for safe loading and unloading of children. Remember that double parking is unsafe and illegal. Teachers and school personnel consistently instruct children to cross at the crosswalks only . That extra minute could prevent a serious or fatal accident. There have been several "near misses" when parents have called children to cross in mid block, between parked cars or even as parents have escorted them across the middle of the street. The police do ticket when they observe jaywalking.

BICYCLE SAFETY - Neighborhood children in grades 4 and 5 may ride their bicycles to school if:

•  Both parent and child have signed a bicycle registration card and it is on file in the school office.

•  The pupil observes all the rules and regulations on the registration card.

•  The bicycle has a current Los Angeles City Bicycle License.

•  The bicycle is in a safe riding condition and has an operable lock.

•  The bicycle is walked onto the school grounds and locked in the bicycle rack.

•  Child owns and wears a helmet. If any of these rules are not observed, bicycle privileges may be suspended.

LOITERING/TRUANCY ORDINANCE - In essence, the ordinance prohibits any person under the age of 18 and subject to compulsory school attendance, from loitering in or upon the public streets, highways, roads, alleys, parks, playgrounds, or other public grounds between the hours of 8:30AM and 1:30PM on days when the student's school is in session.

The exceptions to this are as follows:

•  a student is accompanied by a parent or guardian.

•  a student is on an emergency errand directed by a parent or guardian.

•  a student is going or coming directly from their place or gainful employment.

•  a student is going to or returning from a medical appointment.

•  a student has permission to leave campus for lunch and has in his/her possession a school-issued permit.

•  a student is going to or returning from a public meeting.

If a student is found to be in violation of this ordinance, he/she will receive a citation. This will require you, as the parent, to appear in court with your youngster and may include a fine of $250 or community service. The fine and community service may be suspended only if the student returns to court at the end of 60 days with proof of no unexcused absences for that period. Failure to appear or repeated offenses may result in suspension of, or inability to obtain a driver's license.

The effective date of this ordinance is October 1, 1995, and it will be enforced through the Los Angeles City Traffic Ordinance Citation Program. It is our hope that every student and parent become aware of this ordinance and ensure that all students attend school.

DRUG, ALCOHOL, TOBACCO-FREE AND WEAPONS-FREE SCHOOL - Plainview does not allow the use of drugs, alcohol or tobacco by any person on the school property. There is a zero-tolerance policy in effect for possession of narcotics, drugs, intoxicants, tobacco products (including matches and lighters) and any other substances that may have a harmful effect on the lives of students.

No person, student, staff or visitor may possess a firearm, knife, firecracker or other implement which could cause physical harm to others.

Any student in possession of or using the above will be automatically recommended for expulsion from the school district. In addition to the above, no person on the site (child or adult) will be allowed to sexually assault another individual. This act will also be recommended for expulsion from the school.

FAMILY EDUCATIONAL RIGHTS AND PRIVACY ACT - OPTIONAL - Please read the attached form carefully and return the tear-off portion indicating your preference in the release of information.

STUDENT ACCIDENT INSURANCE - OPTIONAL - Each year children have accidents on the school grounds or on the way to and from school. The school district carries no insurance for the doctor bills involved. Low cost student accident insurance is available through a private insurance company. For your convenience a brochure is attached. Please read this brochure carefully, as some changes have been made this year. If you choose to insure your child, applications should be sent directly to the insurance company .

RESOURCE SPECIALIST PROGRAM - The (RSP) is for children in the regular program with identified learning disabilities. If your child is having a problem, please feel free to discuss it with his/her classroom teacher, who would be the person to initiate a referral.

BILINGUAL VOLUNTEERS - HELP! Parents, students, or community volunteers who speak English and one of the following languages are needed to help in several classrooms. The languages are:

1. Armenian 2. Korean 3. Pilipino 4. Vietnamese 5. Arabic 6.Thai

BILINGUAL PROGRAM - Plainview fully implements the State Law regarding Proposition 227. If you have any questions regarding your child's bilingual program, please contact our office.

TOLL-FREE NUMBER - SECURITY SECTION - ZENITH 2-3456 - Please use this number to report vandalism, burglary, and other acts of suspicious nature during non-school hours.

DRESS CODE

The Los Angeles Unified School District's policy on standards of appearance for pupils (Board Rule 2251) states pupil shall be required to show proper attention to personal cleanliness, health, neatness, safety, and suitability of clothing and appearance for school activities. In every case the dress and grooming of the pupil shall be clean and shall not:

1. Cause actual distraction from or disturbance in any school activity or actually interfere with the participation of a pupil in any school activity.

2. Create a hazard to the safety of himself or others.

3. Create a health hazard.

BASED ON THIS POLICY, PLAINVIEW SCHOOL BELIEVES CHILDREN SHOULD NOT :

•  Wear shoes with high heels, poor traction or open toe, because these can be dangerous for children who

must daily play on the yard and participate in physical education. Comfortable, closed shoes such as sneakers are recommended for student use.

2. Have any body piercing, other than studd earing in the ears.

3. Use offensive or obscene statements on shirts, buttons, bookcovers, etc. Students will be asked to remove these items.

4. Wear hats inside the classroom. Wear headbands on the forehead. They are not to be worn in class or on the playground.

5. Wear short shorts, midriffs, sandals, tube tops, and/or spaghetti strap clothes. We believe children should wear comfortable clothes to school (especially on hot days). However, clothes that are more suitable for the beach do not foster a sense of seriouness about school are not safe and are not acceptable for school use.

6. Wearing make-up to school is not allowed.

7. Backpacks are NOT allowed.

EXPECTATIONS

I. GENERAL SCHOOL EXPECTATIONS:

Students, parents and staff will:

1. Be courteous and respectful.

2. Take responsibility for the cleanliness of the bathroom, campus, playground at all times.

3. Use appropriate respectful, school language.

4. Strive for student achievement.

II. STUDENT EXPECTATIONS:

Students will:

1. Respect the authority of all school personnel.

2. All students will refer to adults as Mr., Mrs., Ms. or Miss.

3. Not bring candy, soda, or gum to school.

4. Walk quietly at all times.

5. Go directly home after classes unless attending an after school activity with parent's permission.

6. Leave radios, toys, markers, etc. at home.

7. Not fight.

III. LUNCH AREA EXPECTATIONS:

Students will:

1. Line up quietly in the lunch line.

2. Will walk and stand appropriately in line at all times.

3. Eat quietly in the assigned lunch area.

4. Respect and cooperate with lunch pavilion supervisors.

5. Remain seated until class is dismissed.

6. Clean up their area and dispose all trash in trash cans.

7. Not remove food from the lunch area.

8. Walk in an orderly manner to the playground.

IV. PLAYGROUND EXPECTATIONS:

Students will:

1. Play in their assigned areas.

2. Follow directions given by all yard duty supervisors.

3. Use the bathroom only with yard supervisors permission.

CONSEQUENCES, DETENTION AND SUSPENSION

I. CONSEQUENCES

If school rules and/or classroom rules are broken, the teacher will:

1. Remind students of school behavior expectations.

2. Warn the student about the problem.

3. Make every effort possible to confer with the parent about the student's behavior and explain detention.

4. Confer with principal and consider appropriate action.

II. DETENTION

1. Teachers may detain a student up to ten (10) minutes without parent's permission.

2. Teacher will make every effort possible to notify parent of detention.

3. Detention notice must be signed by the parent if proposed detention is to be longer that 10 minutes after school.

•  After a warning and one infraction students may be kept after school with written reflection.

5. Three or more infractions may lead to out of class suspension.

6. Teacher will inform parents before out-of classroom suspension occurs.

7. Parents of a suspended student are expected to attend a parent conference with their child's

teacher before student returns to class.

III. REASON FOR SCHOOL SUSPENSION

1. Caused, attempt to cause, or threaten to cause physcial injury to another person.

2. Possession, sale, or furnishing of any firearms, knife, explosive, or other dangerous objects.

3. Unlawful possession, use, sale, furnishing or being under the influence of any controlled substance (DRUGS), alcoholic beverage, or intoxicant.

4. Robbery or extortion.

5. Cause or attempted to cause damage to school or private property.

6. Stolen or attempted to steal school or private property.

7. Tobacco use.

8. Unlawful possession, offering, arranging, or negotiating to sell any drugs or paraphernalia.

9. Disruption of school activities or willful defiance of valid authority of school supervisors, teachers, administrators, or other school personnel in the performance of their duty.

10. Commit any type of obscene or profane act on school grounds. 

IV. CONDITIONS OF SUSPENSION

1. A suspended student can not be returned to their classroom or any other

classroom during the period of suspension.

2. The teacher may require the suspended student to complete any missed

assignments or tests during the suspension.

3. The school district or police department may pursue legal action to recover

damages against student and/or parents or guardians.

•  Parents of a suspended student are expected to attend a parent conference with

administrator and/or teacher before student returns from a school suspension.

INCENTIVES AND REWARDS

I. STUDENT INCENTIVES

Students will be rewarded when:

1. They earn good grades and positive progress reports.

2. They hand in homework daily.

3. They display appropriate behavior at all times during the school day.

II. STUDENTS' REWARD MAY INCLUDE

1. Certificate of recognition for outstanding citizenship.

2. “Student of the Month” award.

3. Notice to parents of good behavior.

4. Class or individual recognition with a small incentive i.e., popcorn, pencils,

ice-cream.
