Pre-Kinder (SRLDP) and Kindergarten Immunization Requirements |

Pre-Kinder Polio: 3 doses DPT: 4 doses MMR: 1 dose, given on or after their 1st birthday Hepatitis: 3 doses HIB: 1 does on or after the first birthday for all children less than 4 years and 6 months. Varicella: 1 dose on or before their first birthday (unless the child has had chicken pox) Mantoux: TB skin test not required for Pre-Kinder
Kindergarten Polio: 4 doses, but 3 are enough if at least one was given on or after the 4th birthday DPT: 5 doses, but 4 are enough if at least one dose was given on or after the 4th birthday MMR: 2 doses, both given on or after the 1st birthday Hepatitis: 3 doses Varicella: 1 dose (unless the child has had chicken pox) Mantoux: TB skin test given and read after June 2004
* As with all California Immunization requirements, children can be admitted as long as they have begun a vaccine series and the next dose is received when due. If the child does not comply with the series of shots, he/she will be excluded from school. No student will be registered for school without the appropriate Immunizations. |
